Bio
Tracy Schrider, LCSW, ACM is currently Community Health Social Service Program Manager focusing on hospital can community partnerships developing innovative programs for complex, vulnerable patients. She is in the Harvard DisparitiesLeadership program through Mass General. Formerly Administrative Supervisor for Social Work at Alta Bates Summit Medical Center. For over 20 years, she served as Medical Social Worker on intensive care, renal and surgery units. She is a member of the Palliative Care Interdisciplinary team rounds and served 14 years on the Alta Bates Ethics Committee. She has a Bachelors of Science in Human Service Studies from Cornell University and her Masters in Social Welfare from UC Berkeley and a Certificate in Theological Studies from the Graduate Theological Union’s Pacific School of Religion. She is currently enrolled at Pacific School of Religion’s leadership development and social transformation program working towards a graduate Masters of Social Transformation focusing on healthcare for the underserved. Tracy and her husband architect Morgan Smith live in Berkeley California and have two teenaged children.

An idea worth spreading
After studying one patient who came to our emergency department 80 times in 6 months, we convened all her supports in the community, got her housing, recovery services, and consistent providers. Her last emergency visit was 18 months ago. Telling stories of patients and tracking progress and communication among hospital and agencies help us understand how we need to improve car and lower costs for vulnerable individuals.
